Registration Dates:
Thursday, July 27 - 9:00-12:00
If your child is going into Kindergarten or new to CPS, please follow these instructions. You will need to bring the following documents:
**2 proofs of current residency (utility bill, phone bill, lease or mortgage, voter registration)
**proof of identification (driver's license or state ID)
**child's original birth certificate or passport (no copies)
**child's up-to-date physical/immunization records/ allergies or other health conditions
**child's up-to-date dental information
**child's eye exam information (kindergarten only)
**2 recent paystubs (Pre-K only)
All medical, dental and eye exam information must be current for the 2023-2024 school year.
If your child is going into 1st, 2nd, 3rd, 4th, 5th, 6th, 7th or 8th grade for the school year 2023-2024 AND they are already CPS students, you will need to bring the following documents:
*2 proofs of current residency (utility bill, phone bill, lease or mortgage, voter registration)
*updated physical/immunization records/ allergies or other health conditions and dental information
**6th graders need a updated physical and immunization records
*proof of identification (driver's license or state ID)
*inform your current school of your plans to transfer so they can "project" the student to NTA.
If you do not have the proper medical documentation by registration dates, please make appointments prior to the registration dates and still register. You will then bring the appointment date documentation with you (email printout or confirmation of appointment).
